Lechem Mishneh, Double Bread for Shabbos

From the parsha: "And on the sixth day they gathered double bread (lechem mishneh) two portions for one."

What does Mishneh mean? Rashi: double or different in terms of flavor and aroma.

When should we commemorate the double bread?
Sons of the land of Israel break bread on Shabbos on one loaf so as not to enter the eve of Shabbos on Shabbos, the sons of Bavel break bread on 2 loaves on Shabbos as they require "double bread"

When should the Lechem Mishneh be required?
Kol Bo- only the first two meals
Rambam: every one of the 3 Shabbos meals
Hag. Maim: every meal on Shabbos

Why Lechem Mishneh?
Kavod of Shabbos, in honor of Shabbos
Zecher leman, to remember the manneh

Which one is cut?
Rashba, Maharshal, Shalah, Gera: both
Aruch Hashulchan: just one
Shulchan aruch: the bottom one
RAMAH: at night on the bottom, on Shabbos and Yom Tov on top

If you cut the bottom one, how do you avoid passing over the mitzva?
TAZ: Leave the bottom one closer to you

Was there lechem mishna on Yom Tov?
Rif: yes
Mechilta: kidsho be-man
Gemara betza re. Hachana: no manna on Yom Tov
Mechilta: Shabbat, lo yihyeh bo: to include Yom Tov
Rambam: Lechem mishneh is required
Pesach: debate
